About Us

Who We Are
MCPN is a nonprofit organization and has provided medical and health education services to the underserved since 1989. Our mission is to provide excellent health-related services focusing on the underserved. Our current service areas encompass Jefferson, Arapahoe, Adams and Park Counties and the City of Lakewood and Aurora. MCPN has one of the largest population base of any Community Health Center in Colorado and, the largest number of underserved individuals in the State.
Overview of Our Services
Services provided in all clinics include well child visits, immunizations, physicals, chronic disease management. Prenatal care is provided at the Jeffco, Englewood and North Aurora Family Health Services clinics. Each of our clinics provides limited pharmacy and lab support as well as the ability to arrange services through referrals. We have staff who take a proactive role in helping patients work through applying for aid from medical programs like Medicaid, Colorado Indigent Care Program (CICP) and Child Health Plan Plus (CHP+). Clinical Initiatives focus on diabetes, asthma, immunization, and Women's health. The Reach Out and Read Initiative allows providers throughout the network to promote childhood literacy by prescribing books to read at each visit
Health Education Services
MCPN provides case manage-ment and health education programs in the following areas:

AmeriCorps service learning Programs for Adults, where health education and outreach services for MCPN patients and clients are provided.

Health Education Classes comprised of Diabetes, Asthma, Childbirth, Smoking Cessation, Family Planning and Healthy Living.

Healthy Start Project, providing care coordination for pregnant women and families with infants up to two years of age in Aurora, Englewood, and Sheridan.

Perinatal Services, creating effective linkages for pregnant women with the MCPN service system and providing Doula Birth Coaches.

Community Network
The Metro Community Provider Network (MCPN) was created in response the indigent healthcare needs of Jefferson and Arapahoe Counties, and the City of Aurora and Lakewood. Our first Clinic doors opened in Aurora in 1989. Since then, we have grown to 10 sites and several community-based case management programs.
